Welcome to the GBJT enrolment form. In completing this enrolment form you agree to the following Terms and Conditions of playing and being a part of the GBJT.

Terms and Conditions

1. All Players and Parents agree to be bound by the Rules, Terms and Code of Conduct of GBJT as contained and updated from time to time on the GBJT web site.
2. Players agree to be available to play for the whole season including when on bye (except when unable to play due to sickness).
3. Parents must be available to supervise their child's court when rostered (usually once or twice a season).
4. From time to time your photos may be taken and placed on the GBJT web site or permission given to media outlets for news articles about GBJT tennis fixtures.
5. All communication to Players and Parents is though email and our web site. Please ensure the email address provided will be read and able to receive PDF attachments.

GBJT Club Tennis Polo Shirt

All Players in Season 2, 2010 will receive a GBJT Head Polo Tennis Shirt. Please click on the preferred size of the shirt required:

Nomination fees and payment details

The cost for Season 1 will be $145 plus the Tennis Queensland 2010 membership fee (if not already paid) of $30 making a total fee of $175 (discount of $10 for each additional family member). Full payment must be received no later than grading day.

All players receive a GBJT hat in Season 1 as part of the nomination fee paid.

Direct Deposit Payment:

Please make payment to Bank of Queensland; BSB: 124029; Account No.: 10617208 and please include the players name so it is recorded on the GBJT bank account.

Cheque Payment:

Please make cheque payable to ‘GBJT’ and mail to PO Box 630, Indooroopilly, QLD, 4068 and please include the players name.
